If you want to pay with a credit card print and fill up this 1 page form.
You can also pay by check but it will delay the shipping of your replacement 7-10 days for check clearing, if check come back unpaid no replacement will be sent and bracelet that we received will not be shipped back.

All you need to do is send $25 instead of $20 with indication of which design would you want to receive.
